EX-SERVICEMEN CONTRIBUTORY HEALTH SCHEME (ECHS) APPLICATION FORM FOR MEMBERSHIP (REV 2015)
1. Ref Central Org ECHS letter No Bl49711/AG/ECHS dt 15 Jan 2015.
2. The application form as fwd vide the letter under ref may please be treated as cancelled. A fresh application form is fwd herewith and the inconvenience caused is regretted.
3. Application form (Revised 2015) for ECHS Smart Card in case of fresh applicants has been modified with additional details. The format for the same is enclosed herewith. The format of the application has also been uploaded on our website www.echs.gov.in. The same may be downloaded.
4. It is mandatory for all applicants to provide details of Aadhar card incl those of dependents. Aadhar card details for NBC ESM is not mandatory.
5. The old application form be discontinued wef 30 April 2015 and the new revised form be implemented wef 01 May 2015.
6. It is requested that the above be given wide publicity and disseminated to all concerned under your jurisdiction.

Sir,
We are the regular visitors for ECHS clinic bangalore at command hospital, for the past 8 years .
My wife is an heart patient, I take medicines only for BP.
As the medicines for the heart patient is the same very rarely changes dosage , like asetrom 1 mg or 2 mg, bp tabs like we most of the times these tabs are not available,
You can drag the details of the perticular patient every year you will come to know about the regular usage of the medicines , accordingly they can purchase and keep the stock for the month.
But it never happes why because every time patient comes from far away places like 25 to 30 kms he cannot come again back for the same medicines when it is availabe ,
One suggestion why not to make available required medicines to regular patients so that problem can be solved .

It’s really apparent that Esm are not treated as it should be . TThere should be provision of seeing specialist if needed. State govt. is providing better facilities. We’re simply become subject of exploitation.
Dear sir, What is the use of ECHS facilities to to the olf and infirm veterans. Every time we approach ECHS doctor the patient is referred to MH where the Army doctor id alread overburdened with Medicals, PMEs and army patients. There is no body to attend to old anx infirms. So ECHS should be scrapped. This is reality of ECHS UDHAMPUR and CH (NC) OPD Deptt. Dr Maj Balki
