Railway Form 6 for Declaring family Particulars : The Railway Form 6 is a statement to be submitted by Employees to declare the eligible family members for the Family Pension Scheme.

Based on the details provided in the Form 6 only the family pension will be paid after the demise of the railway employee

This will be applicable for the employees those who are eligible for Family Pension. Now the family Members of employees covered under New Pension Scheme also eligible for family pension if he/she exercised option for extending death benefits under CCS Pension Rules 1972

The Form 6 to be duly filled and signed by the employee and submit to the concerned head of office or Accounts Officer along with a witness of another Railway employee, and after clear verification by the authority, the submitted application must be kept in records of the service book of the employee.
